Primary Health Care
S creening, monitoring, referral and follow up services help Community Rehabilitation Center consumers get the medical attention they need. Onsite Primary Health Care services include:
- Clients undergo a complete physical with lab work, which screens for chronic diseases such as diabetes, hypertension, heart disease, and kidney functioning. The Duval County Health Department and Agape Community Health Network completes these physicals and lab work.
- If screening detects a potential health problem, clients are referred to a medical facility. If a chronic disease is diagnosed, Community Rehabilitation Center Primary Health Care services will help monitor the disease with a quarterly blood pressure, vital sign and glucose reading check.
- Clients learn ways to improve or maintain good heath and prevent illness. They may learn how to properly use their medication, eat well to avoid generational diabetes, conduct breast exams, exercise and more.

The Community Rehabilitation Center also provides transportation to medical appointments from a consumer's adult living facility or from CRC.The Primary Care staff includes a registered nurse, a licensed practical nurse, a medical assistant and a medical doctor.
